Medical Advisor Covid-19 Emergency Response – Siria
Position Title: Medical Advisor
Location: North East Syria or Remote Work in case of lockdown of the Country
Period: 4 months

Starting date of the project: ASAP or October 2020
Context
The Medical Advisor (MA) will work under the supervision of the Medical Coordinator in the Health Department of UPP NES Mission to ensure the design, implementation, monitoring and evaluation of the activities related to the Intensive Care Units Management and trainings for COVID 19 response in UPP supported facilities in NES.
The MA role is also to advice the Programme Department in the decision-making process and deliver project related documents according to donor requirement, under the supervision of the Programme Manage.

ROLE AND RESPONSIBILITIES.
The medical advisor will be responsible for:
- Providing technical advice and guidance to Programme Department and to Mission Management in the sector of intervention in which the advisor is assigned (ICU management and COVID-19 response)
- Contributing in the decision making processes related to the design and implementation of strategies and tools related to the specific sector of intervention (ICU management and COVID-19 response)
- Developing the functional design and operation of the services of supported facilities
- Assessing, advising and organizing clinical services, distribution and organization of staff, and the overall dynamic flow and roles of the ICU management and COVID-19 related services in supported facilities and provide regular monitoring and follow up
- Contributing to ensure accountability in front of the patients and communities for the services under his/her responsibility
- Training the general staff ascribed to the supported ICU facilities in:
- General care of COVID 19 patient with special dedication to the operative IPC measures in the unit
- General organization of the ICU related to the flow of patient, general services, guarding and cleaning, supporting services, logistics and special procurements and maintenance
- Training of the clinical staff ascribed to the supported ICU facilities (doctors, nurses, paramedics and midwifes) in:
- General clinical methods and treatments for CoVID-19 patient with detailed dedication to organic failure, cardiac and respiratory arrest and assistance, shock prevention and treatment and sepsis prevention and treatment
- Developing expertise inn invasive monitoring and mechanical ventilation
- Organizing and advising a training curriculum for doctors, assistant anesthesia professionals and nurses of ICU, to be developed according to International recognized standards
- Monitoring the overall functioning of ICU services and COVID-19 response in the targeted facilities, meeting the challenges and signaling the gaps and solutions to correct them, through regular field visits (when possible) or by developing and implementing tools for remote monitoring in case physical access is limited
- Advising and collaborating with the Project Manager in any demand related to his/her area of expertise
- Elaborating and analyzing M&E data for reporting purpose – Supporting the project managers in the reporting activities
ESSENTIAL REQUIREMENTS
- Medical degree (Doctor).
- Minimum of two years professional experience as a doctor of intensive care unit
- Accredited experience as trainer or teacher of intensive care services in community bases primary care models.
- Accredited experience in attending critical COVID 19 patients
- Excellent oral and written English. Arabic is an asset
- The medical advisor for ICU management and training, should have expertise and founded medical practice in both directly attending and training other professionals in the attendance of critically ill patients, with proven experience in attending COVID 19 critically ill patients, with expertise in the following critical events:
- heart andcardiovascularfailure – ALS operation
- renal (kidney) failure
- liver failure
- patients with reduced levels of consciousness or neurological impairment (including weakness)
- gastrointestinal failure
- respiratoryfailure and respiratory assistance
- sepsis– a serious immune system reaction triggered by infection
- The medical advisor has to have proven experience in assessment, monitoring and review of patients with or at risk of developing critical illness and the provision of organ system support, including:
- use of inotropes and vasopressors
- mechanical ventilation – using a machine known as a ventilator to assist breathing
- invasive monitoring, including arterial cannulation where a tube known as a cannula is inserted into a blood vessel, usually for the delivery or removal of fluids
- endotracheal intubation – a flexible tube is inserted through the nose or mouth into the trachea for artificial ventilation or for the administration of gases during anaesthesia
- percutaneous dilatational tracheostomy – a minimally invasive procedure where the trachea (wind-pipe) is opened from the front of the neck to enable air to be passed into the lower air passages
- measurement of cardiac (heart) output
- renal (kidney) support using haemodialysis or hemofiltration (techniques used to ‘purify’ the blood of patients with kidney failure
- transfer of critically ill patients for specialist investigations and interventions
DESIRABLE REQUIREMENTS
- Experience in planning and implementation of health programs
- Experience in coordination of humanitarian aid projects
- Excellent capacity to work in team.
- Proven capacities to plan activities successfully targeting results and objectives.
- Ability to supervise and/or mentor the medical staff.
- Previous work experience in humanitarian field.
- Working experience with local counterparts (CSOs, local NGOs, local institutions).
- Experience of planning, organizing and prioritizing work, working under pressure and meeting deadlines.
- Ability to work under pressure in stressful environments
